What is the Premier Passport Program?
The Premier Passport Program is a rewards program for our loyal customers. As a member, you receive one point for every dollar spent on eligible services. You will receive a $25 reward certificate after you have earned 500 points which will be available for use at the end of every month. You may use or accumulate rewards certificate(s) to use for any eligible services. Reward certificates do not expire. Membership is free and easy to join.
How do I join?
Membership registration is easy. Next time you are at Plastic Surgery Associates simply ask to become registered for the Premier Passport Program. You will need a current email address to receive your reward certificates. Or, if you prefer, call Plastic Surgery Associates at 616 451 4500 and ask to be registered. We will mail you your Premier Passport membership card.
How do I earn reward points?
Every time you purchase products or have a skin care service you will earn one point for every dollar spent. Once you have accrued 500 points you will receive via email a reward certificate worth $25 off your next purchase on any of the following treatments and/or products from Plastic Surgery Associates:
- BOTOX® Cosmetic
- Dermal filler (Juvéderm® and Restylane®)
- Laser skin renewal and laser hair removal
- Any skin care treatment
- Skin care products, make-up, and tools
What do I get for my points?
After you receive your $25 reward certificate via email you can use it on your next purchase of treatments and/or products listed below. Redeem your certificate at the time of checkout. Your reward certificate does not expire, so use it any time.

Are there any restrictions?
Reward points cannot be earned from cosmetic surgery or CO2 laser treatments.
Premier Passport Guidelines
- Reward certificates will be awarded at the end of each month.
- Reward certificates are non-transferable.
- Valid for one time use only.
- May not be applied toward the purchase of gift cards.
- When redeeming the reward certificate—a minimum purchase of $25 is required.
- No cash value.
- Not valid on previous purchases.
- Points are earned when services/products are purchased.
- In the event of a return, the points refunded will be reapplied to your premier passport account.
- Plastic Surgery Associates may, at its discretion, terminate or change the Rewards Program at any time.
